If you are a new resident within the LaBrae School community and need to register your child/children for school, you will register your children using FinalForms. You can access FinalForms via the link below. When you get to the landing page, click on the "New Student" link under the Parent Section. Please review the PDF document on registration below for directions on how to create an account and register students. Parents who have had FinalForms accounts in another school district, must create a new account. A FinalForms account from another district will not work.
If your new student is an athlete, we also need:
- A current, valid physical using the OHSAA approved PPE form (available pre-filled once you have set up Final Forms)
- Copy of most recent report card
- AFFIDAVIT FOR BONA FIDE CHANGE OF LEGAL RESIDENCE (for students transferring from another school only)
